Season Information

Below are items needed by by ALL athletes looking to participate in the Boca Hoops Youth Basketball upcoming Season:

1] Birth Certificate (for new players)
2] Proof of residency [new players] :: See below for boundaries
3] Medical physical* (dated the year of the Boca Hoops season being played) confirming players health
4] Child must be present at the time of registration
5] Parent or guardian present to sign registration forms
6] $100.00 Registration Fee (Payable to: BOCA HOOPS, INC.)

*Physicians will be at registration dates to perform physical exams for a $25 Boca Hoops Donation.
